    1. the brand name and model number on the actual furnace do not correspond with the brand name and model number that you supply below AND
    2. the furnace does not qualify for the program.
  2. In connection with the program described on this website, you will be required to sign a release. You may download and print out the release. sign it, and give it to the inspector who is assigned to your claim. If you choose not to download the release, you will be able to obtain the release from the inspector assigned to your claim. By supplying information to this website, you agree that you will execute the release as a condition of receiving the remedies under this program.
